Confidential Client Information and Health History

On your first office visit, you will be asked to complete a form regarding your basic and medical history information. This form is used to assist the therapist in determining how to provide the best possible massage on a case by case basis. If there are certain areas you would like to discuss in detail, then please do so BEFORE your massage session so your therapist will have a full understanding of your condition. If at anytime during your session you feel uncomfortable, please tell your therapist immediately. Your health and well-being is our primary concern.

 NOTE: All client information and medical health history is held in the strictest of confidence. Unless specifically requested by you, your information is not released to anyone for any purpose. The only exception to this rule is as governed by local and Texas state law.

Do Not Schedule For a Massage Session If You:

·         Have the cold or flu

·         Have a contagious rash

·         Have any type of skin condition that can be aggravated by massage

·         Have open sores

·         Bacterial infection
